NSAIDs : Ketorolac

Indication

Ketorin (Ketorolac) is indicated for the short-term management of moderate to severe pain. A beneficial effect has been shown in acute pain, in renal colic and in cancer pain but the major indication is for post-operative pain.

Contraindication

Ketorolac is contraindicated in the following situations: Patients previously sensitive to Ketorolac or Aspirin or other NSAIDs – Patients with active or suspected peptic or duodenal ulcer or who have GI or other bleeding disorders. – In severe heart failure and renal or hepatic insufficiency.

Dosage & Administration

Oral Dose:

10 mg every 4-6 hours (Elderly every 6-8 hours) to a maximum of 40 mg daily for a maximum duration of 7 days.

Parenteral Dose:

For adult patients (<65 years): Initial dose : 60 mg IM / 30 mg IV (single dose), Maintenance dose : 30 mg IM/IV every 6 hours, Maximum dose : 120 mg/day, Duration of treatment : 2 Days. 

For elderly Patients >65 years, renally impaired and/or less than 50 kg of body weight: Initial dose : 30 mg IM / 15 mg IV (single dose), Maintenance dose : 15 mg IM/IV every 6 hours, Maximum dose : 60 mg/day, Duration of treatment : 2 Days.

For Paediatric Patients (2 to 16 years of age): Single IM dose of 1 mg/kg up to a maximum of 30 mg or single IV dose of 0.5 mg/kg up to a maximum of 15 mg.

Side Effect

The major side effects are gastrointestinal like dyspepsia, abdominal pain, nausea, fluid retention, peptic ulceration, pancreatitis, palpitation, bradycardia, purpura etc. Flatulence, gastritis, constipation, vomiting etc may also occur.

Precaution

Ketorin (Ketorolac) should be used with caution in patients sufferingfrom GI disease and history of peptic ulceration, cerebrovascularbleeding, ulcerative colitis, cardiac decompensation, hypertension, renaland hepatic impairment. Dose should be reduced in elderly.

Ketorolac is contraindicated in the later stages of pregnancy and should only be used earlier in the pregnancy if there are compelling reasons. Because Ketorolac is distributed in milk, it should not be used during lactation.

Drug Interaction

Drug interactions may occur with Methotrexate, Diuretics.Antihypertensive drug, ACE inhibitors and Warfarin.

Ketorin tablet : Box containing 2 x 10 Tablets in blister pack.Ketorin-30 Injection : Box containing 1 ampoule in blister pack.
